#PerfectPearls - Hip and Lumbar Spine Physical Examination Findings in People Presenting With Low Back Pain, With or Without Lower Extremity Pain

The etiology of low back pain (LBP) is not well understood. Is it structural, chemical, emotional, a mix of all of the above?
_____
Low back pain, especially chronic low back pain can be difficult to assess at times. Part of the reason is that Pain distributions from lumbar spine disorders can be similar or overlap with disorders of the hip and pelvic girdle.

Not only do these symtoms overlap, but hip and spine symptoms can coexist causing hip-spine syndrome.

The objectives of this study were to:
1️⃣Collect observational cross-sectional data regarding patient characteristics and hip physical examination findings.
2️⃣Compare pain and function in patients with positive hip exam findings with those without them.

Results:
🎓Passive hip flexion and hip IR were reduced in those who had low back pain and a positive hip provocation Test.
🎓Those with LBP without a positive hip provocation Test had normal hip range of motion.
🎓80% of the randomly selected 101 patients had low back pain and a positive hip provocation test.
🎓Those who had positive hip tests complained of greater low back pain and reduced function than those with LBP alone.

Clinical Judgement
✅Evaluation of the low back and hip should be taken into consideration for those with LBP.
✅For those with spine-hip syndrome, a different approach to treatment may be necessary depending on what is the primary driver of pain.
