Dentistry is complex, there are risks with every procedure and if its discussed before its part of consent. If its done after it is seen as an excuse. 

Take ownership of issues that occurs as we are the doctor in charge. If you can solve the issue do so in a way that doesn’t leave the patient worse off. If you can’t then you need to refer the case to someone who can. 

Who pays for mistakes depends on consent. If I discussed it before its a no fault system so the patient doesn’t pay for my time and just pays for materials used (think post op infections and minor implant complications), patient neglect are paid for by the patient in full and my errors/neglect are paid by me (usually by not charging for my time to correct the mistake). These can be from minor bite adjustment issues to implant placement errors and everything in between.
