The film depicts Dr. Atul Gawande’s personal journey as he strives to improve communication with his own seriously ill patients.  Through his individual experience, not only with his patients, but with his own father, he has recognized the value of talking with patients regarding what brings value and meaning to their lives, and how he can assist them, as their physician, with identifying and addressing their own personal goals of care as they approach their final transition.  Open communication, honesty, addressing fears and worries, and determining realistic hopes are key in having these often difficult conversations.
