A Council Bluffs-based provider that took on care of more than two dozen former residents of the closed Glenwood Resource Center has been placed on probation following allegations of abuse of its clients.

The Iowa Department of Health and Human Services placed Community Options on probation effective Oct. 29, meaning the nonprofit community-based residential care provider is prohibited from accepting more clients and must take corrective action to address the deficiencies the state laid out.

According to the state's letter, the organization's clients experienced more than 70 critical incidents and there were more than 30 reports of adult dependent abuse from July 2023 to September 2024.

Community Options has pushed back against the accusations of abuse, calling them "completely unfounded."
