This past June marked two years since the Titan submersible disaster that ended with five lives being lost. After a two-year investigation, the Coast Guard announced its findings and concluded the incident was “preventable.” According to nbcnews, #OceanGate, the company that owned the submersible, had a toxic workplace environment and intimidation tactics that contributed to the fatal incident.

Jason Neubauer, the chairman of the U.S. Coast Guard Marine Board of Investigation, said, “The two-year investigation has identified multiple contributing factors that led to this tragedy, providing valuable lessons learned to prevent a future occurrence.”

Many factors led to the fatal disaster, including OceanGate’s repeated use of the Titan after multiple incidents that compromised the integrity of the hull and the company’s failure to conduct detailed investigations after those problems.

Investigators said the company’s leaders fired senior staff members and threatened other terminations to “dissuade employees and contractors from expressing safety concerns.” They also found that the company “leveraged intimidation tactics” to “evade regulatory scrutiny” and ultimately utilized the Titan “completely outside of the established deep-sea protocols.”

During a test dive days before the fatal incident, Titan crew members encountered multiple challenges, and specialists expressed concerns that the company’s CEO, #StocktonRush, was rushing. They “felt Mr. Rush was beginning to get ‘antsy’ and ‘clearly frustrated’ and that Mr. Rush made the statement, ‘I’m going to get a dive in, even if it kills me,’” the report states.

On June 18 2023, the Titan went on a planned dive to the Titanic wreck. The submersible lost communication with its OceanGate handlers, which led to a search. It was later determined that the submersible imploded and instantly killed all five people on board. (📸: gettyimages) ✍🏾: #TSRStaffJW
