I just finished 7 months working 6 days/week on inpatient psychiatry. Common diagnoses I’ve treated include schizophrenia, substance-induced psychosis, bipolar mania and depression, suicide attempts and ideation, severe depression and/or anxiety, PTSD, and severe personality disorders, mixed with some meth, opioid, cocaine, alcohol and cannabis use disorders.  This job comes with a level of responsibility I’m honestly still getting comfortable with: I’ve held family meetings for new diagnoses of schizophrenia and bipolar disorder, I’ve gone to court countless times as an expert witness to involuntarily commit and medicate patients (which I do not take lightly), I’ve stayed late advocating to discharge patients when I felt the hospitalization was causing more harm than good, I’ve sat and held space for patients who have experienced unfathomable trauma. But through all the difficult conversations and decisions this year, I’ve felt myself grow both in clinical reasoning as a physician, and as a human providing support to other humans.  I also feel so fortunate to be training at an institution on the forefront of psychiatric intervention; having an ECT suite with a mini OR on the unit has been incredible. I’ve seen my patients go from being so severely depressed that they can’t eat or even get out of bed do a 180 after ECT. We also have IV ketamine on the unit for acute suicidality, and I’ve prescribed the brand new antipsychotic Cobenfy for a few patients with treatment resistant schizophrenia since it came to market this year. My experience has solidified my desire to be an inpatient/emergency psychiatrist after residency, so that I can hopefully make psych admissions less traumatizing to those involuntarily committed and be there in general for people in crisis.
