The tryout application is OPEN…. Don’t wait till last minute to fill out your tryout forms‼️

The following requirements are due BY April 17th ⬇️

1️⃣ Physical within the last 6 months performed by a MD, DO, PA-C, or ARNP
2️⃣ Copy of front and back of primary insurance card (insurance information sheet)
3️⃣ Liability Waiver (this cannot be signed electronically)
4️⃣ Sickle Cell Test Results
5️⃣ On Campus Tryout Waiver 
6️⃣ Tryout Application
