⏮️ Reversing the effects of permanent fillers and rigorous steroid treatments.

This face tells a complex story. One that speaks to how unskilled interventions often by untrained, non-medical providers can lead to serious long-term consequences, both physically and psychologically. It’s a powerful reminder of how much is at stake in our field. But these before and after photos also carry something equally important: Hope..

And for me, that’s at the heart of what plastic surgery is about; restoring identity, correcting damage, and helping patients move forward with confidence.

This particular patient had a difficult journey. She received permanent #filler injections 💉 from a non-medical provider, which resulted in #granuloma formation. These were then treated with multiple rounds of high-dose steroid injections, leading to atrophic scarring and significant thinning of the dermis, epidermis, and subdermal fat. Both superficial and deep fat compartments were compromised. Ultrasound even revealed thinning of the facial platysma, which is continuous with the SMAS layer.

With a regenerative, layered true plastic surgical approach and with the help of a specialized device generously made available to me by ecopcu , we worked to rebuild what had been lost.

She underwent two sessions of microfat grafting enriched with mechanically processed stromal vascular fraction (the stem-cell-rich portion of fat). This was combined with:
	•	Two sessions of CO2 laser
	•	Multiple rounds of microneedling to address skin atrophy
	•	Radiofrequency subcision, a technique I learned from the brilliant ecopcu 

Each #fatgrafting session was planned with precise mapping—not just to correct deformities, but to respect and refine her natural contours. In her second stage, we also performed a closed rhinoplasty to bring better harmony and balance to her features.

These are the cases that push us as plastic surgeons not just technically, but emotionally. These type
of special cases require lots of beain storming with experienced passionate colleagues. I feel incredibly fortunate to do this work, and I’m grateful to my colleagues and mentors who continue to support me along the way.
