## Caption

Medicaid, which provides medical care for low-income and disabled Americans, is jointly financed by state and federal governments. In many states, the program is run by the state government. 

But in Iowa, former Republican Gov. Terry Branstad privatized the system beginning in 2016, contracting with for-profit companies like Iowa Total Care to manage portions of the program in an effort to both reduce quickly rising costs and improve quality of care.

Now, 10 years later, Iowa's Medicaid program continues to face budget pressures that are only expected to intensify as surges in pandemic-era funding wane and provisions of President Donald Trump's "One Big Beautiful Bill" begin taking effect.
