Tryout applications are due THIS FRIDAY‼️ Make sure you have ALL requirements submitted to the tryout application by April 17th ⬇️

1️⃣ Physical (within 6 months — MD, DO, PA-C, or ARNP)
2️⃣ Copy of front & back of your insurance card
3️⃣ Liability Waiver (must be signed in person — no e-signatures)
4️⃣ Sickle Cell Test Results
5️⃣ On-Campus Tryout Waiver
