A new federal audit found hundreds of millions of dollars in potentially improper Medicaid payments for autism-related services in four states, raising concerns about fraud and inadequate oversight of providers.

The Department of Health and Human Services audit identified millions of improper or incorrect payments tied to supposed autism care in Indiana, Wisconsin, Maine and Colorado. The audit found up to $600 million in Medicaid benefits were paid out across those four states, with potentially improper billings that included services that did not include therapy and payments for patients who were not properly diagnosed with autism.

Government waste watchdog Open the Books has been tracking the issue. Rachel O’Brien, deputy public policy editor at Open the Books, said the audit found examples of questionable billing practices.

“They found that providers were billed, for billing, rather for children napping or eating, playing on tablets, watching movies when they're supposed to be providing them therapy, essentially,” O’Brien said. “In Colorado, one provider billed for 151 hours of therapy in a single month. We saw staff without credentials delivered therapy across multiple states.”

O’Brien also cited estimated household costs tied to the potentially improper spending in the four states reviewed.
