You hand the pharmacy $100 for your prescription. Ready for the breakdown?

💸$40 to the drug manufacturer.
💸$30 to the PBMs (Caremark, Optum, Express Scripts).
💸$10 to your insurance. Plus rebates on the back end.
💸$20 left for the pharmacy.

But that $20 isn’t profit. $5 goes to labor. $5 to fees (we get charged every time an electronic prescription hits our system. Then DIR fees on top of that). $10 covers the inventory we’re floating for up to 90 days, the rent, the lights.

So your local pharmacy keeps almost nothing. The PBMs keep $30 for being a middleman you’ve never met. And then people wonder why pharmacies are closing on every corner.
