A Tuba City behavioral health clinic received nearly $295,000 from Arizona’s Medicaid program between December 2025 and May 2026 before being removed over what the state calls credible allegations of fraud.

AHCCCS investigators say staff at Sacred Circles of Healing and Wellness used ChatGPT to generate clinical notes that were copied and pasted into records for billing. The clinic is also accused of billing for services that could not have occurred as claimed, including appointments involving patients who allegedly never attended or left early.

Investigators also claim $500 bonuses were offered for bringing Native American patients into the program. AHCCCS says Denis Artiles, who previously operated another terminated provider, was secretly involved in Sacred Circles’ daily operations despite not being disclosed by its CEO.

The clinic, Artiles and CEO Coleen Chatter are now banned from participating in AHCCCS for five years. The Arizona Attorney General’s Office says the matter is also under FBI investigation.

These are allegations and have not resulted in a criminal conviction.
