Longevity is entering its evidence crisis.

Medicare is now giving eligible Americans aged 65 and older access to major GLP 1 obesity treatments for $50 per month.

Many experts expected older patients to choose pills.

Instead, doctors say many are requesting injections because the weight loss results are stronger.

People are choosing efficacy over convenience. 

That could expand treatment to millions who were previously blocked by cost.

But weight loss alone is not healthspan.

Older adults also need to protect:

Muscle

Strength

Bone

Mobility

Hydration

Metabolic resilience

A smaller body is not automatically a stronger body.

At the same time, the FDA is reviewing seven peptides promoted for recovery, performance, cognition, and anti aging.

Compounds such as BPC 157, TB 500, and Semax have spread rapidly through influencers, Reddit, fitness communities, and biohacking culture.

Consumer demand is strong.

Human evidence remains limited. 

That is the new longevity divide.

Institutional medicine demands proof.

Consumer experimentation demands speed.

Forward Hypothesis

The next longevity winners will not simply promise more years.

They will prove that people can preserve strength, cognition, mobility, and independence while living them.

Longevity is becoming public medicine on one side and a permissionless biological experiment on the other.
