This is one of those developments that could look much bigger in hindsight.

Merck and Moderna have announced positive Phase 3 results for intismeran, an individualized mRNA cancer therapy created specifically from the unique genetic mutations found inside a patient’s tumor.

The global trial included 1,137 patients with stage IIB through IV melanoma whose tumors had been completely removed.

Patients receiving intismeran plus Keytruda experienced statistically significant and clinically meaningful improvements in both recurrence free survival and distant metastasis free survival compared with Keytruda alone. (Merck.com)

This is the first positive Phase 3 result for an individualized mRNA cancer therapy.

And there is already long term evidence behind it.

In the earlier Phase 2b study, the combination reduced the risk of recurrence or death by 49% and the risk of distant metastasis or death by 59% after five years compared with Keytruda alone. Those percentages are from the earlier trial, not today’s Phase 3 study. (Merck.com)

The Phase 3 trial is continuing to evaluate overall survival, and the therapy still needs regulatory approval.

But think about where this is heading.

A tumor is sequenced. Its unique mutations are identified. An mRNA therapy is created specifically for that patient. The immune system is then trained to recognize those mutations and attack cancer cells carrying them.

We spent decades talking about personalized medicine.

We are beginning to see what it actually looks like.
