A Fixel/UF case study highlights the challenges of treating essential tremor, a progressive movement disorder that can continue to worsen over time. After experiencing initial relief from MRI-guided focused ultrasound, a patient's tremor returned within a year and began affecting both hands.

Faced with a difficult treatment decision, the care team chose a different approach: deep brain stimulation (DBS). Following staged bilateral DBS surgery, the patient achieved significant tremor control on both sides.

This case underscores an important question in essential tremor care: what is the best path forward when symptoms return after focused ultrasound? While there is currently no clear consensus, the findings suggest that DBS may offer a durable and adjustable option for patients whose tremor recurs.
