Brazil just drew a line at 6 hours. But is the clock really the best way to define surgical safety?

A new rule in São Paulo limits elective cosmetic surgery to six hours, bringing an important conversation to the forefront: How long is too long in the operating room?

As a plastic surgeon, I think the answer is more nuanced than simply watching the clock. Patient selection, the procedures being performed, the surgical team, anesthesia, the facility, and knowing when enough is enough all matter. But there’s really solid science to suggest that six hours is a pretty smart place to draw the line.

There’s a reason I’m willing to tell a patient “no” or recommend staging procedures rather than trying to accomplish too much at once.

Safety should always come before convenience—or checking one more procedure off the list.
