🎯 INTUSSUSCEPTION ✅

In the usual pediatric intussusception, ultrasound demonstrates:

🎯 Target / doughnut sign on transverse imaging
🥪 Pseudokidney / sandwich sign on longitudinal imaging
🔄 Invaginated bowel and mesenteric fat
🩸 Variable mural vascularity on Doppler

Most pediatric cases occur in infants and younger children and are typically ileocolic.

In an older child with small-bowel intussusception, particularly ileo-ileal, a pathological lead point deserves a careful search—Meckel’s diverticulum, polyp, lymphoma, duplication cyst, etc.

Also look for bowel wall oedema, proximal dilatation, altered peristalsis, mesenteric fat changes, free fluid and reduced/absent vascularity suggesting bowel compromise.

And if a persistent intussusception is missed or neglected:

Venous congestion → oedema → ischemia → necrosis → perforation → peritonitis/sepsis.

So, the lesson from this case?

When an older child points to the pelvis, don’t assume the pelvis is the problem. Sometimes, the bowel is telling the story.
