A Mesa family says a hospital released their husband and father’s body to a funeral home they never selected, and by the time they figured out where he was, he had already been embalmed.

Raymond Wood Jr., 54, passed away July 9 at HonorHealth Four Peaks Medical Center. His wife of 32 years, Leslie, says the family contacted several mortuaries before deciding where they wanted him taken.

When Leslie called the hospital to coordinate the transfer, she says she was initially told they didn’t know where her husband’s body was.

She later learned Advantage Funeral & Cremation Services had already picked him up, even though the family says they never chose that funeral home. By the time the mistake was discovered, Raymond had already been embalmed. Leslie also says it was done without his dentures, something she said he never went without.

The family says the funeral home acknowledged the transfer should not have happened and arranged to move Raymond to the mortuary they had actually selected.

HonorHealth told Arizona’s Family the transfer was arranged “based on the information available at the time” and said its process is to work with families when situations like this occur. Advantage declined to discuss the specific case with the outlet.

The family filed a complaint with the state. A spokesperson for Advantage said the investigation found insufficient evidence to substantiate the allegations.

Raymond’s family ultimately chose cremation so they could keep his remains close, but they say they still want to know how he ended up at the wrong funeral home in the first place.

How does a mistake like this even happen?
