## Caption

Buccal Fat Removal - intraoperative footage.  I have already injected lidocaine and made a small incision in the cheek lining at this point.  In the video, I am using an instrument to spread through the buccinator muscle and enter the buccal space, where you can see the buccal fat pad peeking out.  Gentle pressure expresses the fat pad.  I do not remove the entire fat pad to avoid over hollowing - only the portion of the fat pad that is easily expressed.  I will cauterize and cut through the base of the expressed fat, and then use a couple dissolvable sutures to close the incision.  This procedure takes less than an hour and is easily performed in office with oral relaxing medications.
