To hit the fade, move the ball position forward to open your shoulders and influence an out to in path. With your trail hand, grip the top of the grip to open the shoulders, and the club face. To make sure it cuts, take the club away outside, and let the toe drag behind the heel in the downswing.
To hit the draw, start with your 3 wood. The extra loft will help you draw it without sacrificing distance. Then, move the ball back in your stance, and place your trail hand under the grip. This will help close your shoulder, and close the club face. Now, you're set up for an in to out path, and a closed face. In the motion, make a nice full turn, and let your lead shoulder go up in the downswing.
