A “human error”.
That’s what led to sex offender Hadush Kebatu mistakenly being released from a UK prison last week, according to Justice Secretary David Lammy.
Kebatu was serving a 12 month sentence for two sexual assaults, including one on a 14-year-old girl, during his stay at The Bell Hotel in Epping.
Mr Lammy has said an independent review of HMP Chelmsford will now take place and that all prisons in England and Wales will have more checks done on prisoners due to be released.
The extra checks have been criticised by some senior prison staff who told BBC News the extra workload will put pressure on a system already struggling to cope.
