Yes, it can be - but only sometimes (unless you’re doing trauma 24/7 🙃) For the most part, patients are thoroughly worked up prior to their surgery. You know all about their comorbidities and are well prepared to handle the situation if it occurs. For example, if a patient has a known difficult airway - you will have the glidescope ready and fiber optic available for backup. If you need to go down the airway algorithm, it is not a surprise because you already knew it was a high possibility. There are many other scenarios ranging from surgical complications such as massive blood loss to other events like anaphylaxis or pulmonary embolisms. You will learn how to manage all of these during the program and in clinicals.
