If you’ve used a nerve stimulator in the ICU, you’ve probably learned just enough train-of-four to get through a shift.   But in CRNA interviews (and especially in anesthesia) TOF means a lot more than counting twitches.   It’s not just “paralyzed vs not paralyzed.”   It’s about receptor occupancy, depth of blockade, recovery trends, and whether a patient is actually ready to breathe on their own.   And during your shadow days, you’ll see the CRNA use the TOF monitor, and save this post so you’ll have an introduction to what they’re talking about!   When I worked in the MICU I didn’t use muscle relaxants or know what a train of four monitor was. It was a totally foreign concept to me. So if you don’t use muscle relaxants, don’t stress!
