A preliminary report by the National Transportation Safety Board states that a series of communication issues is partly to blame for March's deadly runway crash at LaGuardia Airport. The report finds that one of the Port Authority Emergency Officers told investigators he recalled hearing the words, "stop, stop, stop" on the radio, but did not know who that transmission was intended for. Two pilots were killed in the crash.
