Aberfan, South Wales. A school sat beneath a mountain of coal waste. Warnings had been raised. The springs were on old maps. The National Coal Board was blamed. 116 children died. 28 adults died. Nobody was prosecuted. Then £150,000 was taken from the Aberfan Disaster Fund to help remove the remaining tips — money donated in grief, used to clean up the failure left behind.
