Long-term recovery becomes far more difficult when our systems don’t work together.
People with complex behavioral health needs move between emergency departments, homelessness, incarceration, crisis response, and short-term treatment—often without coordinated support across each system. As a result, the burden of navigating that falls on the person, usually at the moment they’re least equipped to carry it.
This was one of the key themes at our second Vision 2030 Solution Symposium in February of this year. Discussions on the intersection of behavioral health care and homelessness, incarceration, and hospitalization continuously emphasized how fragmented systems continue to drive cycles of instability across California.
We know what works. What’s missing is connecting our systems so that care follows the person, not the program.
