A UK inquiry has found a “complete failure” to protect newborns at the hospital where nurse Lucy Letby was convicted of murdering seven babies.
Inquiry chair Kathryn Thirlwall said some of the deaths at the Countess of Chester Hospital could have been prevented if senior staff had acted sooner. She criticized what she described as dysfunctional management and a lack of understanding of safeguarding procedures.
Thirlwall said her inquiry did not assess Letby’s guilt or innocence, but instead examined whether hospital management responded appropriately to concerns about the deaths.
Three senior hospital managers were arrested last year on suspicion of gross negligence manslaughter and remain under investigation.
