An inquiry has found some baby deaths could have been prevented at the Countess of Chester Hospital where Lucy Letby worked. Inquiry chair Lady Justice Thirwall said there was a "complete failure to protect babies" at the hospital and they will never know how many lives could have been saved. Thirwall blamed "dysfunctional management and governance" at the hospital, and that "no-one seems to have understood safeguarding action is required when a member of staff is suspected of causing deliberate harm". The report includes 17 recommendations, including fitting all cots and incubators at neonatal units with baby monitors and installing CCTV cameras directed at storage units storing insulin. Letby is serving 15 whole-life sentences for murdering seven babies and attempting to murder seven more. She was convicted in 2023 and two attempts to appeal have been denied. #LucyLetby #Thirwall #UK #C4News
