The report from the first phase of the inquiry into the Southport killings has said the attack which left three young girls dead "could and should have been prevented". The inquiry found that the killer Axel Rudakubana was "clearly" an extreme danger. But it said that his parents and a series of state agencies had failed to act sooner over his "escalating behaviour".
