I think sometimes when we get overwhelmed, or stuck in a clinical situation that's complex, sometimes we have to go back to basics.
Remember the H's and T's we went over last week? Those are a good place to start.
Hypo/Hyperkalemia is one of those H's.
So what's the treatment? C BIG K DROP
👉Calcium gluconate or calcium chloride (stabilizes cardiac membrane)
👉Bicarbonate (shifts potassium intracellularly)
👉Insulin with glucose (shifts potassium intracellularly)
👉Glucose (given along with insulin to prevent hypoglycemia)
👉Kayexalate (sodium polystyrene sulfonate) (promotes potassium elimination)
👉Diuretics (such as furosemide) (promotes potassium elimination)
👉Resins (such as sodium polystyrene sulfonate) (promotes potassium elimination)
👉Oxygen (supportive measure)
👉Potassium restriction (avoidance of high potassium foods and medications)
