Understanding your ventilator while working in the ICU is keeeeey.
What are some reasons the peak pressure alarm is going off?
How does PPV affect BP?
What are weaning modes? Which modes can patients initiate breaths?
In the OR, I most commonly use PSVPRO, this is a mode where the patient initiates their own breaths, and we support each breath with pressure support. This allows me to titrate narcotics in nicely.
If the patient requires paralysis, I often SIMV PCV-VG. This is a synchronized setting, where we dial in a pre-set volume. VG stands for “Volume Guarantee”. So this means I can set a tidal volume, and as long as we don’t reach a high pressure, the vent will deliver it.
I know this won’t mean much to you now, but my point is to encourage you to study your vents. We’re constantly adjusting these bad boys AND troubleshooting alarms.
